How to Request a Personal Protection Order (PPO)

Select and complete the Petition and Order that applies to your circumstance. Read the following to determine which petition you need to complete.

Domestic PPOs

The petitioner and respondent are: husband and wife; were husband and wife; have a child in common; have or had a dating relationship; reside, or resided in the same household.

Petition (CC 375) – Fill in Completely
Order (CC 376) – Fill in sections C & D only

Non-Domestic PPOs

The petitioner and respondent have never been husband and wife; resided in the same household together; had a child in common; or had a dating relationship with one another.

Petition (CC 377) – Fill in completely
Order (CC 380) – Fill in sections A & B only

What to Do if the PPO is Granted

  • Visit our Office to:
    • Pick-up your signed true copy of the PPO, and
    • A true copy of the PPO for the “respondent” (the person for whom the PPO was taken against)
  • You must arrange to have the PPO served upon the other party in one the following ways:
    • Personal service by an adult other than yourself
    • Personal service by a process server (see list below) or
    • By certified mail & restricted delivery to the “respondent” the person the PPO was taken against
  • Ensure that the Proof of Service is completed and filed with our Office

What to Do if the PPO is Denied & You Want to Contest

  • Contact our Office for a hearing date and time
  • Complete and file Notice of Hearing on Personal Protection Order Form CC 381 at our Office
  • Complete and file the Circuit Court Praecipe
  • Arrange to have the Notice of Hearing, PPO Petition with Questionnaire, and the Praecipe served upon the other party, by one of the following ways:
    • Personal service by an adult other than yourself
    • Personal service by a process server (see list below)
    • Certified mail & restricted delivery to the “respondent” the person the PPO was taken against
  • You must ensure that the Proof of Service is completed and filed with our Office

How to Modify, Terminate, or Extend a PPO

  • Complete Motion to Modify, Terminate, or Extend a Personal Protection Order (CC 379)
  • Complete a Circuit Court Praecipe (A hearing date will be given to you by our Office.)
  • Complete the Proof of Service and Motion and Notice of Hearing forms (see Forms page).
  • The Respondent can only file a motion to modify or terminate a PPO if the motion is filed within 14 days
    after being served with the PPO. (If beyond the 14 days, contact the our Office for additional information.)
  • The Petitioner can file a Motion to Modify or Terminate a PPO at any time.
  • The Petitioner may file a Motion to Extend the PPO three days before the PPO expiration date.

What to Do if the Respondent Violates the PPO

  • Call the police and advise them that you have a PPO
  • Complete Motion and Order to Show Cause for Violating Valid Personal Protection Order (CC 382)
  • Submit the completed Motion and Order to Show Cause to our Office for the Judge’s review
