Requesting a Certified Copy of an Adoption Order

If you are an adult adoptee or the adoptive parent of an adoptee and are trying to obtain a certified copy of the Order of Adoption, please submit a Petition for Adoption Information and Order (SCAO form PCA 327)

• $20 petition fee
• $10 certified copy fee (you will receive one copy)
• $1.50 per each additional copy requested

Requesting Identifying or Non-Identifying Information From Adoption File

Within 63 days after a request for non-identifying and/or identifying information, the Court will provide the nonidentifying and/or identifying information available to the requesting party. Upon receipt of a written request for identifying information, the Court will submit a clearance request to the Central Adoption Registry. Within 28 days after receipt of a clearance reply, the Court will notify the adoptee in writing of the identifying information to which the adoptee is entitled, or if the identifying information cannot be released, the reason why.
