Records Request

There are several ways to request immunization records:

• Ask your family doctor.
• Submit an MDHHS Immunization Record Request.
• Submit an LCHD Record Request.

To request immunization exemption records, use the LCHD Record Request Form. The exemption must be from the current calendar year. LCHD does not have access to historical exemptions.
