The Livingston County EMS CP model uses paramedics in specialized roles to perform basic, nonemergent, clinical tasks in people’s homes to reduce hospital admissions and readmissions, decreasing emergency room visits and extending health care access to populations lacking the insurance coverage, transportation, or ability to establish relationships with their own primary providers.

Livingston County EMS Community Paramedicine:

  • Does not duplicate or compete with existing established community services.
  • Reduces costs to patients and stakeholders by providing standard community health care services outside of the emergency department.
  • Bridges gaps in local health care systems and improves population health and safety.
  • Is not a one-size-fits-all approach and instead offers health care services tailored to community needs.

To submit a community paramedic patient referral please complete the form below. Our Community Paramedicine department will follow-up on referrals within 3 business days. Please note referrals require a physician and/or designee and referring agency.
